Sunday, September 13th is National Grandparents’ Day. On this special day, we celebrate, thank, and remember the grandmothers, grandfathers and other special seniors in our lives who have served as our caretakers, role models, and friends.
Seniors should also use this day as an opportunity to talk with their loved ones about how they can help their special seniors coordinate their care. This is very important considering that more than two-thirds of Medicare beneficiaries have two or more chronic conditions, such as diabetes, heart disease or kidney disease, and often have more than one doctor treating them. Care coordination can help seniors stay healthy by avoiding medical errors, reducing hospitalizations, and improving their quality of life.
